STUDY OBJETIVE: To showcase the 'Hotdog in a Bun' technique, an accessible yet underutilized laparoscopic ovarian pexy method, applicable as a primary or prophylactic intervention after adnexal detorsion.

DESIGN: The video features the case of a 23-year-old, nulliparous woman with an unremarkable medical and surgical history. She underwent urgent laparoscopic adnexal detorsion, immediately followed by prophylactic oophoropexy utilizing the 'Hotdog in a Bun' technique to prevent recurrent torsion within the same operative setting.

SETTING: This surgical case was performed as an emergency procedure within the on-call service at the Hospital Italiano de Buenos Aires, Argentina. The intervention was carried out collaboratively by a third-year Obstetrics and Gynecology resident and the attending gynecologist on call.

INTERVENTION: This video meticulously details the 'Hotdog in a Bun' technique, a less commonly described method for ovarian suspension to the round ligament [1]. The presentation outlines the step-by-step surgical intervention and highlights the subsequent positive clinical outcomes.

CONCLUSION: The 'Hotdog in a Bun' technique is a simple, effective, and readily reproducible method for ovarian pexy, suitable for gynecologic surgeons proficient in basic laparoscopic suturing. This technique holds significant clinical application for ovarian preservation post-detorsion, particularly in cases without predisposing risk factors [1]. It preserves normal tubo-ovarian anatomy and maintains the ovary in a physiologic pelvic position, supporting future fertility [2,3]. It can also serve as a prophylactic measure for securing the contralateral ovary, aiming to reduce recurrence rates (reported 11%-18%), thereby significantly reducing the overall risk of recurrent ovarian torsion [4,5].
