BACKGROUND: Early detection of high-risk varices (HRV) in patients with compensated advanced chronic liver disease (cACLD) is crucial for improving prognosis. We aimed to compare the diagnostic accuracy of liver stiffness (LS) and spleen stiffness (SS) measured by two-dimensional shear wave elastography (2D-SWE) and transient elastography (TE) in the diagnosis of HRV, and to develop an HRV diagnostic model based on 2D-SWE for the patients with cACLD.
METHODS: From October 2023 to March 2025, 104 consecutive adult patients with cACLD (defined as TE measured LS ≥ 10 kPa) without previous decompensation were prospectively enrolled. All patients underwent TE and 2D-SWE examinations, laboratory tests and esophagogastroduodenoscopy (EGD) at enrollment.
RESULTS: The area under the receiver operating characteristics (AUROCs) (95% CI) of TE-based LS and SS for HRV were 0.706 (0.533-0.879) and 0.917 (0.853-0.982), with optimal cut-off value of 23.6 kPa and 46.0 kPa, respectively. The AUROCs (95%CI) for 2D-SWE based LS and SS were 0.717 (0.545-0.889) and 0.790 (0.684-0.897), with the cut-offs of 20.7 kPa and 35.5 kPa, respectively. We developed a diagnostic model using 2D-SWE-based SS and spleen thickness (ST), which demonstrated non‑inferior performance compared to established non‑invasive models.
CONCLUSIONS: TE-derived SS and our new non-invasive model based on 2D-SWE are useful for HRV diagnosis in patients with cACLD.
