INTRODUCTION: The objective of this study was to evaluate the periodontal support tissues of maxillary central incisors before and after orthodontic treatment by measuring the periodontal ligament area (PDLA) using a cone-beam computed tomography (CBCT)-periodontal ligament (PDL) automatic segmentation network.

METHODS: The CBCT-PDL segmentation network established in our previous research was employed to segment and measure the PDLA of 131 maxillary central incisors from 66 patients. The changes in PDLA before and after orthodontic treatment were calculated. Additionally, Dolphin (version 11.9; Dolphin Imaging and Management Solutions, Chatsworth, Calif) orthodontic measurement software was used to measure changes in root length (RL) and the occurrence of fenestration and dehiscence. Statistical analysis was conducted to examine the correlations among these factors.

RESULTS: After orthodontic treatment, the incidence of PDLA reduction in maxillary central incisors was 96.2%, with a median reduction of 8.75 mm2. The percentage reduction in PDLA was 5.8%. Root resorption occurred in 90.1% of patients, with a median RL reduction of 0.68 mm (5.8%). When RL reduction exceeded 2.0 mm or 10.0%, the reductions in PDLA and its percentage increased significantly. The reduction in PDLA showed a significant positive correlation with root resorption (r = 0.473; P <0.001). The group with fenestration or dehiscence exhibited significantly higher PDLA reduction and percentage reduction compared with the group without these conditions (P <0.001).

CONCLUSIONS: We proposed a new method for evaluating changes in periodontal support tissue around a tooth before and after orthodontic treatment: PDLA measured by a CBCT-PDL automatic segmentation network. The reduction in PDLA is closely related to root resorption and alveolar bone loss, making it a clinically significant indicator for evaluating changes in periodontal support tissues.
