Аннотация

Intraorbital foreign bodies (IOFBs) represent significant challenges in ocular trauma management, particularly when complete burial occurs. We present a male in his 50s who sustained a high-velocity ocular injury by a completely buried IOFB consisting of an electric cutter blade fragment. Despite wearing protective eyewear, the 34×11 mm fragment entered through the nasal root and became embedded in the frontal process of the zygomatic bone. Preoperative cerebral angiography confirmed the absence of ophthalmic artery injury. A lateral orbitotomy approach using Berke canthotomy and controlled osteotomy of the frontal process allowed safe foreign body extraction. This allowed the successful removal of the electric cutter blade fragment with complete globe preservation. Although the patient's visual acuity deteriorated over time, excellent cosmetic outcomes with maintained globe morphology were achieved at 1-year follow-up. This report emphasises the importance of multidisciplinary collaboration between plastic surgery and neurosurgery for the management of similar cases of IOFB burial.
