OBJECTIVE: To compare the temporomandibular joint (TMJ) disk status, osseous change of the condyle, and condylar bone remodeling before and after orthodontic treatment with or without a TMJ positioner, and to evaluate the relationship between TMJ positioner use and TMJ changes.
STUDY DESIGN: The subjects were 160 joints of 80 patients with malocclusion as well as suspected TMJ disorder and underwent orthodontic treatment. They were divided into TMJP and No-TMJP groups based on whether they used a TMJ positioner during retention after a full-contact appliance for a couple of months and subsequently dynamic orthodontic treatment. Pre- and post-treatment disk status, osseous change of the condyle, and bone remodeling were assessed by MRI.
RESULTS: Bone remodeling occurred significantly more frequently in the TMJP group (P = .0093). In the No-TMJP group, post-treatment disk status changes differed significantly between joints with reduction and without reduction (P = .0216). Pretreatment disk condition did not significantly influence bone remodeling in either group, and bone remodeling showed no significant association with post-treatment disk status changes.
CONCLUSIONS: Orthodontic treatment with a TMJ positioner tended to promote bone remodeling compared with treatment without it. However, other adjunctive devices and the degree of patient cooperation may also play a role.
