Individualized surgical approach for Brodie's syndrome with facial asymmetry: a case report involving mandibular segmentation.
Аннотация
Brodie's syndrome is a rare transverse malocclusion, challenging to manage, particularly in adults with significant skeletal involvement. This report describes an unusual clinical case of surgical treatment for a unilateral presentation of Brodie's syndrome using mandibular segmentation, an approach rarely documented in the literature. The patient, an 18-year-old, presented with mandibular deviation, Class II malocclusion, and posterior scissors bite, with an adequately wide maxilla but evident mandibular constriction. Three-dimensional planning guided a bimaxillary surgical intervention that included mandibular segmentation into three parts, advancement genioplasty, xenogeneic bone grafting, and correction of maxillary cant through Le Fort I osteotomy. The integration of virtual planning and patient-specific guides enhanced the predictability of outcomes and minimized risks. After 12 months, a stable functional occlusion and significant esthetic improvement were achieved. This case highlights the value of mandibular segmentation as an effective and safe alternative in selected cases of Brodie's syndrome of mandibular origin.
