A novel device for sealing dural perforation via an epidural needle: an in vitro evaluation of effectiveness.

BACKGROUND: Postdural puncture headache is a debilitating complication of unintended dural puncture. There is no currently available solution for sealing the perforation at the time of the puncture.
METHODS: An in vitro model using fresh porcine dura secured to a vertical tube was established. Dural perforations were created with 16G, 17G or 18G Tuohy needles. Untreated perforations served as controls. In the intervention groups, a compressed collagen sponge plug was deployed through the Tuohy needle immediately after puncture. Leakage was assessed by measuring the time for the saline pressure to drop from 20 cm H2O to 10 cm H2O in controls, and by progressively increasing pressure to a maximum of 90 cm H2O in sealed samples.
RESULTS: Saline leaked rapidly in all control groups at 20 cm H2O. The sealing device successfully prevented leakage at 90 cm H2O in 17 of 18 samples. The single instance of leakage occurred in the 17G group at a pressure threshold of 50 cm H2O, with a minimal leakage rate of 6-7 drops min⁻1 at 90 cm H2O.
CONCLUSION: In vitro studies have shown that this simple collagen-based sealing device can be delivered through an existing epidural puncture needle and can effectively reduce saline leakage. It offers a promising new approach that may reduce the incidence of post-dural puncture headache.
