INTRODUCTION: Autologous biofillers are emerging blood-derived regenerative treatments for soft tissue augmentation and regeneration. Despite increasing use, there is limited consensus regarding their definition, preparation, mechanisms, indications, efficacy, longevity, and safety.
OBJECTIVES AND HYPOTHESIS: To systematically review the literature, providing an evidence-based overview of biofillers and informing clinical practice. We hypothesize that blood-derived biofillers are a safe and effective regenerative treatment for facial soft tissue augmentation.
STUDY DESIGN: A systematic review of the literature.
METHODS: Studies evaluating autologous blood-derived biofillers for facial soft tissue augmentation were included. Data on definition, preparation, indications, outcomes, longevity, and safety were synthesized narratively.
RESULTS: Twenty studies were included. Biofillers were primarily used for facial rejuvenation, infraorbital restoration, wrinkle reduction, and skin improvement. Clinical outcomes and patient satisfaction were favorable, while adverse events were predominantly mild and transient. Heterogeneity in terminology, preparation, and outcome reporting precluded meta-analysis.
CONCLUSION: Autologous biofillers are promising, with encouraging clinical outcomes and a favorable safety profile, but further studies are required to define their role in aesthetic practice.
