Аннотация
BACKGROUND: Lip mucosal purpura is rare in Sjögren disease (SjD) and is easily overlooked without classic sicca symptoms, posing a diagnostic challenge.
CASE DESCRIPTION: A patient had persistent oral red macules for 7 years. Serology revealed antinuclear antibody, anti-SSA/Ro, and anticentromere antibody positivity, meeting SjD criteria. This prolonged clinical course before diagnosis underscores the challenge in identifying SjD with atypical manifestations. Treatment included hydroxychloroquine immunomodulation and supportive oral care. This case underscores that isolated oral lesions can be a long-standing indicator of underlying SjD.
PRACTICAL IMPLICATIONS: Lip mucosal purpura, a rare but distinctive SjD sign, may represent an initial manifesting sign. This case highlights the value of oral manifestations in screening for systemic autoimmune diseases and facilitating timely intervention.
