BACKGROUND AND OBJECTIVES: The anti-CD20 therapy is an efficacious therapy for neuromyelitis optica spectrum disorder (NMOSD) through depletion of B cells. The purpose of this study is to examine the effects of B cell depletion on circulating T cells.

METHODS: We enrolled 40 healthy controls (HC), 18 untreated patients with NMOSD (UNT) and 38 NMOSD patients with anti-CD20 therapy (aCD20) in this study. Frequency of circulating T cells of HC, UNT and aCD20 were assessed in this observational study by multiparametric flow cytometry. In addition, the correlation between T cell subsets and B cells in patients with NMOSD was analyzed.

RESULTS: This study showed that the central memory T (TCM) cells decreased and the effector memory T (TEM) and CD20dimTEM cells increased in NMOSD patients with anti-CD20 therapy. It also showed that TCM and CD20dimTCM cells were significantly positively correlated with CD20+B cells and CD4+TEM and CD20dimCD4+TEM cells were negatively correlated with CD20+B cells. Finally, the study pointed out that the decrease of CD4+TCMTh17.1 and CD4+TCMTh17 cells, as well as the increase of CD4+TCMTh2 cells, were associated with B-cell depletion.

CONCLUSION: B cells and T cells interact with each other in NMOSD. Our research provides new evidence on the effect of B-cell depletion therapy on T cells, and highlights a potential role for TCM, TEM, CD20dimTCM, CD20dimTEM and CD4+T helper cells. The above changes in T cells may also contribute to the therapeutic effect of anti-CD20 therapy in NMOSD.
