AIM: To evaluate the influence of baseline keratinized mucosa width (KMW) on clinical and radiographic outcomes following the surgical treatment of peri-implantitis. The primary outcome was probing pocket depth (PPD) reduction; secondary outcomes included changes in bleeding on probing (BOP), mucosal recession (REC), and marginal bone level (MBL).
MATERIALS AND METHODS: This systematic review and exploratory meta-regression was registered in PROSPERO (CRD420250499589). Electronic and manual searches of PubMed, Embase, and Scopus were conducted up to January 2026, complemented by hand-searching of relevant journals. Prospective and retrospective studies with ≥ 6-month follow-up after surgical peri-implantitis treatment reporting baseline KMW and clinical outcomes were included qualitatively. Only prospective clinical trials were included in the quantitative analysis. Data were extracted at subgroup level, standardized, and analyzed using weighted linear regression models (sample-size and inverse-variance weighting) to explore associations between baseline KMW and outcome changes at 6 and 12 months. Risk of bias was assessed using RoB 2 and ROBINS-I tools.
RESULTS: Seventeen studies (12 prospective, 5 retrospective) were included qualitatively. The quantitative analysis comprised only the prospective trials, representing 622 implants and 503 patients distributed across 27 subgroups. No significant association was found between baseline KMW and changes in PPD, BOP, or MBL at 6 or 12 months. Regression models showed flat slopes and very low R² values, indicating minimal explanatory power. Conversely, greater baseline KMW was significantly associated with increased mucosal recession at 6 months (p = 0.04; R² = 0.48), though this was not confirmed at 12 months.
CONCLUSION: Within the limitations of subgroup-level data and study heterogeneity, baseline KMW did not influence improvements in PPD, BOP, or MBL following surgical peri-implantitis treatment. A moderate association with increased soft-tissue recession was observed at 6 months but not sustained at 12 months. Patient-level analyses are warranted to validate these findings and clarify the prognostic role of peri-implant keratinized mucosa.
