OBJECTIVES: To evaluate the changes in anxiety and depressive symptoms following onabotulinumtoxinA (BoNT-A) treatment in patients with facial nerve palsy (FNP) sequelae.

METHODS: This retrospective study included 20 patients with FNP sequelae treated with BoNT-A. Anxiety, depression, and quality of life (QOL) were assessed using the State-Trait Anxiety Inventory (STAI), Self-Rating Depression Scale (SDS), and Facial Clinimetric Evaluation (FaCE) scale at baseline and 1, 3, and 6 months post-injection. To address pharmacodynamic variability, the best score at 1 or 3 months was defined as the "post-treatment peak." Correlations between clinical factors (age, disease duration, and facial grading scores) and therapeutic outcomes were analyzed using Spearman's rank correlation.

RESULTS: At the post-treatment peak, significant improvements were observed in FaCE (p = 0.0029), SDS (p = 0.0032), and STAI-State (p = 0.0014) scores compared to baseline. However, these improvements returned to baseline levels at 6 months. STAI-Trait scores remained stable throughout the study. Older age was significantly associated with limited improvement in depressive symptoms (r = 0.59, p = 0.0061), and severe baseline synkinesis was negatively correlated with the magnitude of improvement in facial-related QOL (r = -0.47, p = 0.038).

CONCLUSIONS: BoNT-A treatment effectively but transiently improves physical symptoms and secondary psychological distress, including reactive anxiety and depression. Older age and severe synkinesis may limit therapeutic benefits, highlighting the importance of comprehensive management and maintenance therapy to sustain mental well-being.
