Application of the Auersvald hemostatic net in arteriovenous fistula revision to prevent hematoma formation.

Postoperative hematoma is a frequent complication of arteriovenous fistula (AVF) revision, threatening wound healing and access patency. The Auersvald hemostatic net is commonly used in rhytidoplasty for hematoma prevention. We report its innovative application in a high-risk patient undergoing AVF revision under supratherapeutic anticoagulation, with aneurysmal degeneration and friable tissue from light chain amyloidosis. Given the elevated bleeding risk and drain displacement concerns, the net was applied after hematoma evacuation to obliterate dead space and provide mechanical hemostasis. No postoperative hematoma developed, and dialysis resumed at 5 weeks, suggesting utility as a drain alternative in select high-risk patients with an AVF.
