Autologous Hematopoietic Cell Transplantation (AHCT) for the Treatment of Newly Diagnosed Multiple Myeloma.

For the past 25 years, autologous hematopoietic cell transplantation (AHCT) has been the cornerstone of therapy for newly diagnosed fit patients with multiple myeloma. However, improved drug therapy, which allows for high rates of remission and even minimal residual disease negativity, calls into question the necessity for the continued inclusion of AHCT in the initial treatment paradigm. This review examines the historical role of AHCT with an analysis of the most recent phase III trials incorporating AHCT, with the goal of interpreting the contribution of AHCT to outcomes. Finally, we examine ongoing clinical investigations that are directly comparing AHCT with other modalities, either immunochemotherapy-based or those integrating T cell redirecting therapy.
