Oculomotor disorders and nystagmus.
Аннотация
The precision of visual perception depends on accurately directing the fovea to objects of interest, a process mediated by six supranuclear oculomotor systems that all converge in the brainstem: saccadic and smooth pursuit, vestibular optokinetic, fixation, and vergence. This chapter provides a clinically oriented overview of brainstem neuroanatomy relevant to eye movement control and links it to characterisitic patterns of gaze disturbance. After outlining the organization and functions of the oculomotor (III), trochlear (IV), and abducens (VI) nuclei and their connections via the medial longitudinal fasciculus, the brainstem substrates for horizontal and versitcal gaze are discussed. A review of the circuitry and physiology of saccades, smooth pursuit, the vestibulo-ocular and optokinetic reflexes, fixation micro-movements, and vergence, emphasizes how shared and parallel pathways support adaptive, stable gaze in dynamic environments. Finally, specific lesion in the midbrain, pons, medulla, and at the cerebella-medullary junction are discussed and how they give rise to distinct forms of nystagmus and gaze palsy. This provides a powerful localization approach in conditions such as demyelinating, vascular, degenerative, and vestibular disorders. By integrating systems-level physiology with detailed brainstem anatomy and clinical approaches, this chapter offers a framework for understanding oculomotor disturbances and for using eye movement examination as a high-yield tool for brainstem localization and diagnosis.
