Comparative analysis of psychosocial well-being and patient satisfaction following blepharoplasty and blepharoptosis surgery.

PURPOSE: To investigate the relationships among psychosocial well-being and patient satisfaction in individuals undergoing blepharoplasty and blepharoptosis surgery, identify the potential predictors of satisfaction, and provide patient selection recommendations.

METHODS

In this prospective longitudinal observational study, 100 patients who underwent either blepharoplasty (n = 77) or blepharoptosis correction (n = 23) were included. The same surgeon performed all operations. Preoperative and postoperative assessments were conducted using the Body Image Disturbance Questionnaire (BIDQ), the Depression Anxiety Stress Scales (DASS), the Rosenberg Self-Esteem Scale (RSES), and the FACE-Q subscales (Appraisal of Upper Eyelids, Adverse Effects, and Satisfaction with Eyes).

RESULTS

Both groups showed significant postoperative improvements in BIDQ, DASS, and RSES scores. Postoperative BIDQ and DASS depression values were significantly lower in the blepharoplasty group (p < 0.001 and p = 0.039, respectively). Appraisal of upper eyelids RASCH scores were significantly greater in the blepharoplasty group during the preoperative and postoperative periods (p = 0.039 and p = 0.008, respectively). FACE-Q scores indicated higher satisfaction in the blepharoplasty group. Preoperative BIDQ (r = -0.25, p = 0.012) and postoperative adverse effects (r = -0.57, p < 0.001) were inversely correlated, and preoperative and postoperative appraisal of upper eyelids (r =  +0.35, p < 0.001; r =  +0.47, p < 0.001, respectively) were positively correlated with satisfaction with the eyes.

CONCLUSION

Upper eyelid surgeries significantly improve body image, emotional well-being, and self-esteem. Patient satisfaction is influenced by both clinical outcomes and subjective perceptions. Appropriate patient selection is essential for patient satisfaction in oculoplastic surgery. Performing BIDQ may help evaluate psychological factors.
