RATIONALE AND OBJECTIVES: This systematic review and meta-analysis endeavors to elucidate the prognostic value of 18F-FDG PET/CT-derived biomarkers, including maximum standardized uptake value (SUVmax), metabolic tumor volume (MTV), as well as total lesion glycolysis (TLG), in the follicular lymphoma (FL) population across different disease stages.
MATERIALS AND METHODS: PubMed, EMBASE, the Cochrane Library, and Web of Science were thoroughly searched from establishment to April 7, 2025. Encompassed studies were methodologically prospective (5) or retrospective (11) cohort investigations into the links of PET/CT parameters to clinical outcomes in FL.
RESULT: A total of 2313 patients from 16 studies were entered. MTV was significantly associated with progression-free survival (PFS) (HR=2.91, 95% CI: 2.20-3.85, P<0.001) and overall survival (OS) (HR=4.30, 95% CI: 2.15-8.60, P<0.001). TLG demonstrated even stronger prognostic performance for PFS (HR=4.92, 95% CI: 2.80-8.62, P<0.001) and OS (HR=9.71, 95% CI: 3.47-27.20, P<0.001). SUVmax was significantly associated with PFS (HR=2.14, 95% CI: 1.50-3.06, P<0.001), but not OS (P=0.086).
CONCLUSION: MTV and TLG are robust independent prognostic indicators of both PFS and OS in FL, whereas SUVmax demonstrated limited predictive utility and was primarily indicative of PFS. Therefore, metabolic tumor burden metrics can be integrated into the risk stratification frameworks of FL. Further validating standardized cut-off values and incorporating molecular biomarkers are necessary to enhance prognostic precision.
