Аннотация

BACKGROUND: Cervical kyphotic deformity is a recognized complication following posterior cervical spine surgery, particularly after laminectomy. However, post-laminoplasty kyphosis remains an underreported entity. We present a case of progressive symptomatic cervical kyphotic deformity following laminoplasty for intramedullary tumor resection.

CASE DESCRIPTION: A 36-year-old male presented with progressive cervicalgia and radicular symptoms in the right arm. Magnetic resonance imaging (MRI) revealed an intramedullary hemangioblastoma at C5 with an associated cyst from C3 to C6. The patient underwent C5-C6 laminoplasty and complete tumor resection with neuromonitoring, the surgery was uncomplicated. After surgery, he had slight numbness in his right hand [modified Japanese Orthopaedic Association Score (mJOA) 17/18]. Three months after surgery, he developed a progressive cervical kyphotic deformity associated with persistent neck pain and dropped-head posture. Conservative management with bracing failed. Surgical correction with a 360° spinal reconstruction-anterior cervical discectomy and fusion (ACDF) (C4-C7) and posterolateral fusion (C4, C5, C7), was performed (11 months after his initial surgery), without any peri-operative complications. The postoperative course was uneventful with complete resolution of neck pain. The same numbness in his right hand remained (mJOA 17/18), without any interference in daily life.

CONCLUSIONS: While laminoplasty is intended to preserve cervical stability, this case demonstrates that clinically significant post-laminoplasty kyphotic deformity can occur even in patients outside the traditional risk groups. We provide a technical framework for 360° cervical reconstruction, emphasizing the importance of intraoperative mobilization techniques and long-term sagittal surveillance in all posterior intradural tumor resections.
