[Research progress on the biomechanical effects of lumbar spondylolysis on adjacent segment intervertebral discs and treatment strategies].
Аннотация
This study systematically reviews the biomechanical effects of lumbar spondylolysis on adjacent intervertebral discs and surgical intervention strategies. By integrating biomechanical experiments, finite element analyses, and clinical data,we found that lumbar spondylolysis significantly increased the biomechanical burden on adjacent discs, leading to reduced stiffness, abnormal stress distribution, and expanded range of motion. Comparative analysis of surgical methods revealed that direct repair techniques effectively reduced adjacent segment stress, while fusion surgery stabilized the affected segment but increased adjacent disc stress. Dynamic stabilization systems (e.g.,segmental spinal correction systems) may mitigate adjacent segment degeneration by preserving partial kinematic function. Personalized treatment strategies should integrate anatomical parameters(e.g.,lumbar lordosis angle, pelvic incidence) and biomechanical characteristics, supported by computational models to predict long-term outcomes. Future research should focus on molecular mechanisms and novel biomaterials to enhance surgical precision.
