Аннотация
BACKGROUND: Conventional TKA prostheses often fail to match Asian knee morphology, where smaller anteroposterior diameters and wider mediolateral dimensions are common. The Meril Destiknee™ posterior-stabilized (PS) system was developed to address these anthropometric differences, while conserving bone and facilitating deep flexion. This study aims to report the mid-term to long-term outcomes of the Destiknee™ PS system focusing on outcomes, implant survivorship and complications.
METHODS: This was a retrospective observational study of patients undergoing primary TKA with the Destiknee™ system between February 2010 and December 2018 at a single high-volume centre with a minimum 5-year follow-up focusing on functional outcomes (OKS, ROM), implant survivorship and complications. Kaplan-Meier survival analysis was performed with revision as the endpoint.
RESULTS: Of 1238 TKAs performed, 1109 were eligible for analysis with a mean follow-up of 7.6 years (range, 5.6-14.4). Mean age was 63.1 years, and 69.3% were female. OKS improved significantly from 20.8 ± 7.1 to 37.1 ± 6.1 (p < 0.001), while ROM improved from 82.4° ± 22.2 to 108.3° ± 14.3 (p < 0.001). There were 41 reoperations (3.7%), of which 23 were major revisions (2.1%). The most frequent indication was infection (52.2%). Kaplan-Meier analysis projected a cumulative 10-year implant survivorship of 97.9%.
CONCLUSION: The Meril Destiknee™ system demonstrated excellent mid- to long-term survivorship and significant functional improvement, with survivorship comparable to other contemporary TKA designs. Multicenter studies with extended follow-up are needed to validate these findings across diverse populations and practice settings.
