OBJECTIVE: Temporomandibular joint replacement (TMJR) is performed for end-stage TMJ disease when conservative management fails. Early postoperative restriction in mouth opening is common due to swelling and evolving fibrosis. This study evaluated the feasibility, safety, adherence, and preliminary effectiveness of early telehealth-supported rehabilitation using Restorabite, a novel jaw-stretching device, following TMJR.

METHODS: Seventeen patients undergoing TMJR were enrolled in this prospective single-centre feasibility study. Participants commenced Restorabite therapy early postoperatively and were assessed at baseline, 10 weeks, 6 months, and 12 months. Primary outcomes were maximum incisal opening (MIO) and trismus-related quality of life (Gothenburg Trismus Questionnaire, GTQ). Secondary outcomes included swallowing- and speech-related quality of life (MDADI, EAT-10, SHI, MGQSF). Feasibility measures included adherence and telehealth participation.

RESULTS: Mean MIO increased by 14.4 mm at 10 weeks (P < .001) and 20.8 mm at 12 months (P < .001). GTQ improved by 39.8 points at 10 weeks (P < .001), with sustained gains at follow-up. Adherence was strong, with 59% completing the prescribed dose, no device-related adverse events, and 100% telehealth attendance.

CONCLUSION: Early telehealth-supported rehabilitation using Restorabite after TMJR is feasible, safe, and associated with meaningful improvements in mouth opening and trismus-related quality of life, warranting evaluation in controlled trials.
