PURPOSE: To measure edema-defined area at risk (AAR) and infarcted myocardium in reperfused acute myocardial infarction (AMI) patients by T1 rho of cardiac magnetic resonance (CMR) and predict adverse left ventricle (LV) remodeling in the convalescent stage.
METHODS: Sixty patients diagnosed with AMI and treated with PCI underwent CMR and all of whom had a follow-up CMR scan after three months. Cine imaging, T1 rho, T1-Mapping, T2-Mapping, T2-weighted STIR and LGE imaging were performed.
RESULTS: Among AMI patients, 37 (61.7%) patients had microvascular obstruction (MVO). Both edema-defined AAR and infarcted myocardium can be detected by T1 rho. The cut-off value between edema-defined AAR and intact myocardium was 62, the paired differences between T1 rho and T2-STIR were: (Mean: 0.5; SD: 5.1, 95% CI: -3.2-6.3). The cut-off value between edema-defined AAR and infarcted myocardium was 80, the paired differences between T1 rho and LGE were: (Mean: 0.3; SD: 2.4, 95% CI: -4.1-4.3). At follow-up CMR scan, baseline infarcted size by T1 rho was related to the follow-up presence adverse LV remodeling according to uni- and multivariable regression analysis. According to receiver operating characteristic curve (ROC), the baseline T1 rho infarct zone value can predict adverse LV remodeling at 3-month follow-up (Area under the ROC (AUC): 0.808, cutoff value: 80 ms).
CONCLUSIONS: T1 rho can distinguish edema-defined AAR from both infarct zone and remote zone. Baseline infarcted zone T1 rho value was related to follow-up infarcted size and MVO presence, also, with appropriate threshold, T1 rho value can predict adverse LV remodeling at follow-up.
