Late-presenting acetabular fractures: Strategic principles, decision-making, and management.
Аннотация
Acetabular fractures occur after high-energy injuries and are often challenging for the treating orthopedic surgeon because of the complexity of the fracture pattern and associated injuries. Timing of surgery is very important and has a direct impact on the outcome. However, in developing countries, lack of infrastructure, limited access to trained pelvic surgeons, delayed referral, and associated life-threatening injuries can result in delay in definitive management. Hence, delayed presentation of acetabular fractures is a major concern when aiming for optimal outcomes. By around three weeks, biological changes such as callus formation and soft-tissue contracture reduce fracture mobility and obscure fracture planes, making reduction significantly more difficult. The displaced wall or column fragments and associated femoral head injuries are difficult to reduce anatomically. Unlike fresh fractures, indirect reduction of the opposite column is often not possible in delayed cases. Hence, these injuries frequently require extensile or combined approaches to achieve a congruent hip. Decision-making becomes more important than technical execution in these cases. Not every fracture is suitable for reconstruction, and careful patient selection is essential. Even after successful reconstruction, long-term outcomes may be affected by complications such as nonunion, avascular necrosis of the femoral head, heterotopic ossification, and post-traumatic arthritis. In many cases, especially when there is significant articular damage, dome impaction, or poor bone quality, total hip arthroplasty may be required either as a primary procedure or as a salvage option. Recent advances in implant design, surgical techniques, and three-dimensional planning and printing have improved preoperative understanding, implant selection, and overall outcomes in these complex cases.
