OBJECTIVE

To report a patient of infected mandibular fracture nonunion managed using the Masquelet induced membrane technique (MIMT), followed by successful implant-supported dental rehabilitation.

METHODS

A 3-stage surgical protocol was implemented. The first stage consisted of radical debridement of infected and necrotic tissues, removal of failed internal fixation devices, stabilization with a reconstruction plate, and implantation of a polymethyl methacrylate bone cement spacer to induce membrane formation. In the second stage, the cement spacer was removed, and the resulting mandibular defect was reconstructed using a composite graft composed of autologous iliac cancellous bone and a synthetic bone substitute. The third stage involved removal of the reconstruction plate, vestibuloplasty, and placement of dental implants after confirmation of satisfactory bone regeneration and infection control.

RESULTS

Postoperative radiographic examinations demonstrated successful reconstruction of the mandibular defect, with progressive formation of cortical and trabecular bone. The regenerated bone exhibited adequate volume and quality to support dental implant placement. All implants achieved satisfactory primary stability and subsequent osseointegration. No recurrence of infection or major complications was observed during follow-up, and both masticatory function and mandibular contour were effectively restored.

CONCLUSION

This report demonstrates that the Masquelet-induced membrane technique is an effective strategy for controlling infection and promoting bone regeneration in complex mandibular defects. It provides a reliable biological foundation for subsequent implant-supported rehabilitation, highlighting its potential clinical applicability in oral and maxillofacial reconstruction.
