Editorial Commentary: Revision Lateral Ankle Ligament Reconstruction Improves Symptoms, but Outcomes Reflect the Revision Context.
Аннотация
Chronic lateral ankle instability commonly develops after a lateral ankle sprain injury. When nonoperative treatment fails, the modified Broström procedure remains the gold standard surgical treatment, with consistently high reported success rates. Nevertheless, a proportion of patients have recurrent instability and require revision surgery. Lateral ankle ligament reconstruction (LALR) is one of the available revision options, but evidence remains limited. Recent literature showed that the primary modified Broström procedure consistently produces superior functional and pain outcomes at midterm follow-up, with more patients achieving the minimal clinically important difference, compared with revision LALR. Findings should not be interpreted as evidence against revision LALR, but rather as a reflection of the different environment in which revision surgery is performed. Revision LALR can still provide clinically meaningful improvement, and patients should be counselled, with expectations managed to the revision context rather than benchmarking it against the outcomes of successful primary repair.
