Аннотация

Opioid use disorder is a treatable chronic disease, yet access to medications for opioid use disorder remains uneven, particularly in rural and underserved communities where prescriber shortages, travel burdens, and restrictive scope-of-practice laws delay care. Community pharmacists are highly accessible medication experts who already monitor therapy, counsel patients, identify safety concerns, and coordinate care. States should authorize pharmacists to initiate or prescribe medications for opioid use disorder, consistent with federal authority and established standards of care. Pharmacy-based care models suggest that this approach can shorten treatment delays and strengthen continuity while supporting individualized care throughout recovery. Expanded pharmacist authority also has implications beyond health care: untreated parental opioid use disorder contributes to child welfare involvement, and timely treatment can help stabilize families and prevent unnecessary separation. Recent federal guidance, Title IV-Efinancing pathways, and the Rural Health Transformation Program give states tools to align pharmacy regulation, Medicaid payment, behavioral health systems, and child welfare policy. Removing unnecessary barriers to pharmacist prescribing could expand the treatment workforce, improve access close to home, and make evidence-based care available when patients are ready to begin recovery.
