Аннотация

BACKGROUND

Guided bone regeneration (GBR) is essential for rehabilitating alveolar defects, with success relying on biological processes and surgical precision. Despite advances in biomaterials and surgical protocols, operator-dependent variability remains a challenge. Integrating microsurgical principles and surgical microscopy-with magnification (5-30×) and coaxial illumination-enhances submillimeter precision, potentially improving accuracy and predictability in GBR procedures. We demonstrated the clinical application of microscopic GBR in challenging periodontal scenarios, highlighting the benefits of magnification, microsurgical principles, and biologically driven regenerative strategies.

METHODS

Three clinical cases requiring GBR in different surgical contexts were analyzed. Case 1: GBR in the mandibular left second molar region (tooth 37) following bone loss due to an endodontic lesion. Case 2: Bone regeneration in the maxillary left first molar region (tooth 26), ridge preservation and vertical GBR at the maxillary left second molar (tooth 27); periodontal tissue regeneration addressing a defect in the maxillary left third molar (tooth 28). Case 3: Horizontal and vertical GBR in the right mandibular first and second molar regions (teeth 46 and 47). All cases were managed using a microsurgical approach under an operative microscope for GBR and soft tissue regeneration.

RESULTS

Favorable outcomes, characterized by significant bone gain, optimal tissue integration, and minimal postoperative complications, were observed. The microsurgical approach enhanced vascularization, preserved soft tissue, and stabilized regenerative results.

CONCLUSIONS

Microsurgical GBR advances regenerative periodontal and implant therapy, especially in complex cases, supporting predictable outcomes, minimizing morbidity, and enhancing soft and hard tissue integration. Its long-term benefits require confirmation.

KEY POINTS: Guided bone regeneration (GBR) is based on four fundamental biological principles: primary wound closure, adequate blood supply, clot stability, and space maintenance, all of which are essential for successful alveolar bone reconstruction. GBR involves replacing lost tissues with biomaterials that restore function and structure, enabling optimal three-dimensional dental implant placement. Surgical microscopy in GBR enhances precision in tissue handling, preserves anatomy, minimizes trauma, improves graft stability, accelerates healing, and facilitates predictable regenerative outcomes.
