The anterior cruciate ligament (ACL) and anterolateral ligament (ALL) play key roles in maintaining rotational stability of the knee joint. Injuries that cause ACL tears have been shown to also frequently compromise the integrity of the ALL. Patients with laxity of the ALL are predisposed to increased rotation of the tibia and subsequent meniscal injury after an ACL reconstruction (ACLR). If such meniscal injury does occur, performing an ALL reconstruction (ALLR) at the time of meniscal repair will potentially limit the extremes of internal tibial rotation, protecting the meniscal repair to allow for healing. This technique describes an arthroscopy-assisted type 2 posterior horn lateral meniscus transtibial root repair and concurrent inside-out lateral meniscus horizontal cleavage tear repair with an open ALLR utilizing iliotibial band autograft following a previous ACLR.
