Аннотация

Rollover extremity trauma can result in severe injuries like open or closed fractures, neurovascular and muscle damage or decollement (soft-tissue degloving). After severe blunt trauma, the risk of compartment syndrome (CS) must also be considered. Early clinical signs of CS are pain out of proportion and increased pain with passive stretching of muscles within the affected compartment. To avoid irreversible muscle or nerve damage, early detection of CS is necessary. However, measurement of compartment pressure for detection is discussed controversially. Its reliability depends on technical skills and patient conditions like blood pressure and comorbidities. In this article, we report the case of a 15-year-old boy who sustained a rollover injury to the thigh. We describe the multidisciplinary management, including endovascular treatment, and discuss the risk of CS following extremity trauma as well as available diagnostic approaches.
