OBJECTIVE: To evaluate the association between monocyte-to-high-density lipoprotein cholesterol ratio (MHR) and pregnancy continuation beyond 13 + 6 weeks in women with unexplained RPL and no previous live birth.

STUDY DESIGN: This retrospective observational cohort study included women aged 18-45 years with unexplained RPL, no previous live birth, at least two prior pregnancy losses, and laboratory measurements obtained at 43-59 gestational days. The primary outcome was pregnancy continuation beyond 13 + 6 weeks. MHR was the primary biomarker of interest; neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, and systemic immune-inflammation index were secondary indices. Multivariable logistic regression adjusted for maternal age, body mass index, gestational age at sampling, number of prior pregnancy losses, and smoking status.

RESULTS: A total of 439 women were included: 145 in the early-loss group and 294 in the ongoing-pregnancy group. Baseline clinical characteristics were similar between groups. Mean MHR was higher in the early-loss group than in the ongoing-pregnancy group (0.01127 ± 0.00486 vs 0.01008 ± 0.00280, p = 0.007). In adjusted analysis, each 0.001-unit increase in MHR was associated with a 9% increase in the odds of pregnancy ending before 13 + 6 weeks (adjusted odds ratio 1.09, 95% confidence interval 1.03-1.15, p = 0.003). Other inflammatory indices were not associated with outcome group.

CONCLUSION: Higher MHR was independently associated with failure of pregnancy continuation beyond 13 + 6 weeks in women with unexplained RPL and no previous live birth. These findings suggest that MHR may reflect inflammatory balance in early gestation and may have value as an adjunctive biomarker in this setting.
