INTRODUCTION: Alcohol and heroin are classified as central nervous depressants. Their addiction has a strong genetic component, although the specific genetic variations involved are not fully understood. This study investigates the roles of alcohol metabolic enzyme genes (ADH1B and ALDH2) in patients with alcohol use disorder (AUD) and heroin use disorder (HUD) that co-occur with AUD.

METHOD: The study involved 1802 unrelated Han Chinese participants: 600 with AUD, 603 with HUD, and 599 healthy controls. Four polymorphisms in ADH1B (rs1229984, rs1042026) and ALDH2 (rs671, rs886205) were genotyped. Stratified analyses assessed genetic patterns based on family history of AUD, and HUD patients with comorbid AUD.

RESULTS: All four ALDH2 and ADH1B polymorphisms were strongly associated with AUD, but not with HUD. After adjustment for age and sex, ALDH2 rs671-A carrier is a marked protective effect against AUD(p = 1.37 × 10-30), whereas ADH1B rs1042026-T and rs1229984-C carriers showed a risk effect (p = 7.15 × 10-12, and p = 1.31 × 10-12, respectively). Stratified analyses showed that HUD patients with comorbid AUD exhibited genotype distributions intermediate between the AUD group and the control group, whereas those without comorbid AUD resembled controls.

CONCLUSION: ALDH2 rs671-A, ADH1B rs1042026-C, and rs1229984-T carriers are important protective factors against the development of AUD. However, these strong protective effects show weakness in HUD with comorbid AUD, suggesting that alcohol-metabolizing pathways do not influence the development of HUD.
