INTRODUCTION: The involvement of syncytiotrophoblast-glycocalyx (STB-GCX) in placental perfusion remains unclear. In this study, we investigated whether the STB-GCX is altered in preeclampsia (PE) and whether its expression correlates with clinical indicators of hypertension and reduced placental blood flow.
METHODS: In a cohort of 61 pregnant women {late-onset PE(LOPE), n = 25; uncomplicated controls, n = 36}, placental perfusion was assessed by measuring the Flow Index (FI) using three-dimensional vocal power Doppler sonography to estimate placental blood flow. Maternal circulating levels of angiogenic factors, including soluble fms-like tyrosine kinase-1 (sFlt-1), soluble endoglin (sEng), placental growth factor (PlGF), and syndecan-1, were also evaluated as biomarkers of PE. STB-GCX expression was quantified via image analysis using syndecan-1 immunohistochemistry. We compared STB-GCX scores and angiogenic factor levels between the two groups and analyzed correlations among STB-GCX scores, FI, angiogenic markers, and clinical findings characteristic of LOPE, including blood pressure, proteinuria, and fetal growth restriction.
RESULTS: The LOPE group had significantly higher sFlt-1 and sEng levels and lower PlGF and STB-GCX scores (2.56 vs. 4.1, p = 0.0222). STB-GCX scores positively correlated with FI (r = 0.3300, p = 0.0094) and negatively with systolic and diastolic blood pressure (r = -0.3278, p = 0.0099; r = -0.2856, p = 0.0257).
CONCLUSION: Reduced STB-GCX levels are likely associated with placental malperfusion and the severity of LOPE.
