Objective:To explore the influence of the body roundness index (BRI) on residual dizziness (RD) in patients with benign paroxysmal positional vertigo (BPPV). Methods:A total of 137 patients with BPPV were enrolled and divided into an RD group(n=44) and a non residual dizziness (NRD) group(n=93). Differences in demographic characteristics, comorbidities, and disease feature were compared between the groups. Multivariate logistic regression and restricted cubic spline models were used to analyze the relationship between BRI and residual dizziness. Results:Compared with the NRD group, patients in the RD group had significantly higher BRI, a higher prevalence of diabetes and history of motion sickness, longer disease duration, and required more treatment sessions (P<0.001). Multivariate analysis revealed a significant positive association between BRI and residual symptoms (fully adjusted model OR=3.04, 95%CI 1.56-5.92, P=0.001). The restricted cubic spline model further confirmed this positive correlation (P overall=0.002) with no evidence of nonlinearity (P for nonlinearity=0.290). Subgroup analyses showed that the association remained consistent across different semicircular canal, gender, and recurrence status, with no significant interactions observed (P for interaction >0.05). Conclusion:Elevated BRI is significantly associated with residual dizziness in patients with BPPV, suggesting that BRI may serve as an important indicator for assessing the risk of poor vertigo recovery.
