Аннотация
INTRODUCTION: Arterial hypertension (AH) is common during pregnancy and in the post-partum period. It is often complicated by pre-eclampsia or eclampsia, with a risk of progression to chronicity. The aim of this study was to determine the prevalence of this persistence beyond 3 months after delivery, and the factors associated with it.
METHODS: This was a cohort study (one arm) with an analytical aim and prospective collection among women received for gravido-puerperal hypertension in the medicine and gynecology-obstetrics departments of the regional hospital center of Kaya. They were followed up to 3 months after delivery. The evolution of blood pressure figures was regularly monitored every two weeks. The gestational or postpartum hypertension was declared cured when systolic blood pressure˂140mmHg and diastolic blood pressure˂90mmHg were recorded at two successive checks spaced two weeks apart. After the descriptive analysis, a logistic regression made it possible to investigate the factors associated with the persistence of hypertension in the postpartum period beyond 3 months after delivery.
RESULTS: A total of 119 women aged 16 to 49 participated in the study with a mean age of 26.18±7.74 years. The age group of 20 to 30 was the most represented (42%). They were housewives (69.75%), uneducated (60.5%). In total, 75.63% had given birth vaginally, 19.33% had a history of cured postpartum hypertension, 22.69% had achieved less than 4 prenatal follow-up, 49.58% had pre-eclampsia, 10 .92% had eclampsia and 29.41% had dyslipidemia. The prevalence of persistence of gravido-puerperal hypertension was 22.69%. The factors associated with its persistence beyond 3 months were: age>30 years (3.73 [1.26-11.17]; P=0.0169), the number of procedures>4 (4, 44 [1.43-14.18]; P=0.0100) and pre-eclampsia (3.3 [1.12-10.26]; P=0.0260).
CONCLUSION: The prevalence of persistent gravido-puerperal hypertension beyond 3 months postpartum is high. Addressing the associated factors in healthcare interventions will help to reduce it.
