INTRODUCTION: The objective of this study is to explore addiction medicine physicians' and addiction psychiatrists' perspectives on prescribing methadone for opioid use disorder (OUD) to be dispensed at community pharmacies.

METHODS: Board-certified addiction medicine/psychiatry physicians (n = 87) in North Carolina completed an online Qualtrics survey from April to July 2025, with a purposive nested sample (n = 10) completing interviews between June and July 2025 via Zoom. We report descriptive results of the survey and emergent themes from interviews.

RESULTS: Attitudes toward prescribing methadone for OUD to be dispensed at community pharmacies were positive. The most highly endorsed benefits reported in the survey were reduced barriers to care for complex patients (65.5%), and increased access to methadone in the community; interviews revealed an additional perceived benefit as having the potential to reduce stigma associated with treatment (63.2%). The most highly endorsed negative consequence reported in the survey was misuse/diversion; yet, interviews elucidate that prescribers are concerned about misuse/diversion for all controlled substances, not methadone alone. Alternatively, the most highly endorsed barriers were lack of a nearby pharmacy willing to dispense and lack of community pharmacist training on methadone dispensing. Interview results reveal that increased communication between the pharmacy and prescriber would be necessary to facilitate implementation.

CONCLUSION: Physicians with addiction specialty training are willing to prescribe methadone to be dispensed to community pharmacies and hold positive attitudes toward pharmacy-based dispensing.
