ETHNOPHARMACOLOGICAL RELEVANCE: Zaogong Erteng Decoction (ZGETD) is a compound Chinese medicine used for endometritis treatment. It not only alleviates inflammation but also promotes the repair of endometrial damage. Nonetheless, the underlying mechanisms remain unclear.

AIM OF THE STUDY: We investigated the underlying mechanisms of ZGETD in treating endometrial damage.

METHODS: The chemical composition of ZGETD was determined using UPLC-HRMS. Endometritis model in mice was induced by lipopolysaccharide, followed by oral administration of ZGETD or amoxicillin. The pathological changes in uterine tissues, the expression of inflammatory factors, the mating rate, the pregnancy rate, and the number of embryo implantation sites were evaluated. Proteomic analysis of uterine samples from the amoxicillin and ZGETD groups was conducted to identify differentially expressed proteins.

RESULTS: Forty main components were identified in ZGETD. Similar to amoxicillin, ZGETD markedly alleviated the pathological changes in uterine tissues, significantly reduced the levels of inflammatory factors, and elevated the mating rate. However, the pregnancy rate of female mice and the number of embryo implantation sites after ZGETD treatment were significantly elevated compared with amoxicillin treatment. The proteomic analysis detected 87 differentially expressed proteins between the amoxicillin and ZGETD groups. Specifically, 60 proteins were downregulated and 27 proteins were upregulated in the ZGETD group. These proteins modulated pathways involved in cell proliferation, apoptosis, vascular regeneration, and extracellular matrix remodeling. The function of these pathways was validated. It was found that the decrease in endometrial fibrosis may be closely related to the active components kaempferol and quercetin in ZGETD, which act on the JAK2/STAT3 signaling pathway.

CONCLUSION: ZGETD may promote endometrial development, reduce endometrial cell apoptosis and fibrosis, and enhance microvascular development, endometrial remodeling, and embryo implantation. It may serve as a more promising treatment for premenopausal women with endometritis instead of antibiotics.
