AIM: To determine the prevalence of pulmonary hypertension (PH), identify associated factors, and evaluate outcomes after adenotonsillectomy in children and adolescents with adeno-tonsillar hypertrophy (ATH) in a low-resource setting.

METHODS: We retrospectively analyzed 1500 patients ≤ 20 years undergoing adenotonsillectomy for ATH in Hawassa, Ethiopia (2022-2025). Pulmonary pressure was estimated via tricuspid regurgitation or pulmonary acceleration time; right ventricular function was assessed with TAPSE and RVMPI. Logistic regression identified PH predictors, and the Friedman test compared pre- and 12-week postoperative measurements.

RESULTS: PH prevalence was 5.9%. Independent predictors included wasting, overweight/obesity, AN ratio > 0.75, daytime mouth breathing, stertor, and obstructive sleep apnea. Among 89 affected patients, 8 had longitudinal and 6 global RV dysfunction. PH and RV dysfunction resolved by 12 weeks post-surgery (P < .001).

CONCLUSION: PH affects a subset of children with ATH but is showed significant improvement following adenotonsillectomy, emphasizing early detection and surgical intervention.
