BACKGROUND: Cognitive impairment is a core feature of schizophrenia and bipolar disorder (BD), yet few studies have explored its association with specific psychopathological symptoms.

AIM: We aimed to analyse the relationship between psychotic symptoms and cognitive impairment using bedside cognitive tests in patients with schizophrenia and BD.

METHODS: This cross-sectional study included 100 patients (50 with schizophrenia and 50 with BD) from a tertiary care psychiatry department. Participants were selected based on inclusion and exclusion criteria and assessed using standardized psychopathology scales and cognitive tests.

RESULTS: Abstract thinking was more intact in BD (72%) than schizophrenia (30%) (P<0.05). Impairment was associated with delusions and hallucinations. Patients with schizophrenia showed poorer semantic fluency than those with BD (P=0.01). Delusions were linked to reduced semantic fluency, while auditory hallucinations were associated with poorer verbal memory and concentration.

CONCLUSION: Specific cognitive deficits are associated with psychopathology, suggesting shared neurobiological mechanisms and potential clinical markers.
