Аннотация

BACKGROUND

As the utilization of hemocoagulase becomes increasingly prevalent, particularly in patients with hemoptysis, the associated coagulative disorders are attracting the attention of clinicians. The risk factors of hemocoagulase-associated hypofibrinogenemia are still not well understood. Our study evaluated the risk factors of acquired hypofibrinogenemia in hemoptysis patients undergoing systemic treatment with hemocoagulase.

METHODS

We conducted a retrospective analysis of the medical records of hospitalised patients who had received hemocoagulase therapy for hemoptysis. Hypofibrinogenemia was characterised by plasma fibrinogen concentrations diminishing below 2.0 g/L. To identify risk factors, we employed logistic regression analysis, and risk assessment standards were pinpointed through applying the receiver operating characteristic curve.

RESULTS

Our study included a total of 60 cases, comprising 27 in the acquired hypofibrinogenemia group and 33 in the non-hypofibrinogenemia group. A higher rate of intensive care unit (ICU) admissions was observed in patients from the hypofibrinogenemia group (22.2% vs. 3.0%; P=0.04). Additionally, those in the hypofibrinogenemia group presented a greater propensity towards massive hemoptysis as opposed to their non-hypofibrinogenemia counterparts (44.4% vs. 18.2%; P=0.03). Cumulative administration time ≥9 days and total hemocoagulase doses ≥6.5 Klobusitzky units (KU) were identified as risk factors for acquired hypofibrinogenemia.

CONCLUSIONS

Acquired hypofibrinogenemia is a common side effect of hemocoagulase use in patients with haemoptysis. Higher total doses of hemocoagulase and longer duration of hemocoagulase administration were identified as risk factors for acquired hypofibrinogenemia in hemoptysis patients.
