Clinical Efficacy of Negative Pressure Suction Combined With Digital Flexible Ureteroscopic Holmium Laser Lithotripsy in the Treatment of Infectious Renal Calculi and Its Effect of Perioperative Inflammatory Indicators.

OBJECTIVE

This study aimed to investigate the clinical efficacy of negative pressure suction combined with flexible ureteroscopic holmium laser lithotripsy (FUHLL) for treating infectious renal calculi and its effect on perioperative inflammatory indicators.

METHODS

A total of 150 patients with infectious renal calculi admitted to our Department of Urology from January 2023 to December 2025 were retrospectively selected and divided into an observation group (79 cases) and a control group (71 cases) based on the surgical methods used. After propensity score matching, 126 valid cases were finally included, with 63 cases in each group. The control group received conventional digital FUHLL, while the observation group received additional negative pressure aspiration. Perioperative indicators, stone clearance rate, inflammatory indicators at different time points and postoperative complications were compared between the two groups.

RESULTS

The observation group had significantly shorter operation duration, postoperative bed rest time, and hospital stay time than the control group (p < 0.05). The stone clearance rate at 4 weeks postoperatively was 93.65% in the observation group, significantly higher than the 85.71% in control group (p < 0.05). At 6, 24, and 72 hours after surgery, levels of inflammatory indicators in the observation group were significantly lower than those in the control group (p < 0.05). The total postoperative complication rate was 4.76% in the observation group, significantly lower than the 20.63% in the control group (p < 0.05). Maximum stone diameter was identified as an independent risk factor for postoperative complications (OR = 16.923, 95% CI: 1.985-144.325, p = 0.010).

CONCLUSIONS

Negative pressure suction combined with digital flexible ureteroscopic holmium laser lithotripsy effectively improves stone removal efficiency and shorten operation and recovery time.
