BACKGROUND: Recognition of sinus echo beats on surface electrocardiography is exceedingly uncommon and may be confounded by sinus arrhythmia or premature atrial complexes with similar P-wave morphology.
CASE SUMMARY: A 29-year-old postpartum woman presented with intermittent palpitations. A standard 12-lead electrocardiogram demonstrated sinus bradycardia with sinus arrhythmia and 2 premature atrial complexes exhibiting distinct electrophysiological behavior. Careful cycle-length analysis revealed that the first premature atrial complex reset the sinus node, whereas the second mimicked complete interpolation, did not reset the sinus node, and was followed by early sinus activation occurring approximately 40 ms earlier than expected relative to the immediately preceding P-P interval, a pattern suggestive of a sinus echo mechanism.
DISCUSSION: This case illustrates a rare and subtle electrocardiographic phenomenon in which early postectopic sinus activation mimics complete interpolation and challenges differentiation from sinus arrhythmia or sequential premature atrial activity.
TAKE-HOME MESSAGE: A nonresetting premature atrial complex mimicking complete interpolation and followed by early sinus activation may be suggestive of a sinus echo mechanism.
