Brain-targeted Therapies for Irritable Bowel Syndrome: Mechanisms and Strategies.
Аннотация
Irritable Bowel Syndrome (IBS) is increasingly recognized as a complex disorder of the gut-brain axis, characterized by bidirectional dysregulation between the enteric and central nervous systems. Advanced neuroimaging has revealed that IBS patients exhibit distinct structural and functional brain alterations, particularly in regions governing pain processing, emotional regulation, and salience detection, such as the Anterior Cingulate Cortex (ACC), prefrontal cortex (PFC), and insula. These maladaptive neuroplastic changes are driven by mechanisms including visceral hypersensitivity, central sensitization, and neuroinflammation. This review synthesizes current evidence regarding brain-targeted therapeutic strategies designed to reverse these central abnormalities. We examine the neurobiological mechanisms of psychological interventions, such as Cognitive Behavioral Therapy (CBT) and Gut-Directed Hypnotherapy, which appear to strengthen top-down inhibitory control over hyperactive limbic circuits. Furthermore, we explore the central effects of acupuncture, which modulate functional connectivity in thalamo-cortical networks, and specific probiotics, which may attenuate amygdala hyperactivity via the gut-brain axis. Emerging pharmacological approaches, including central neuromodulators, are also discussed in the context of synaptic plasticity. The review highlights that effective IBS management extends beyond peripheral symptom control to the potential restoration of neural homeostasis. While current interventions show promise in normalizing brain function, future research must focus on longitudinal studies to identify neuroimaging biomarkers that can predict treatment response, paving the way for personalized, brain-driven precision medicine in gastroenterology.
