BACKGROUND: Levodopa/carbidopa intestinal gel (LCIG) is an established treatment for advanced Parkinson's disease (aPD). Real-world data describing the longitudinal trajectories of specific adverse events (AEs) remain limited.

OBJECTIVES: First, to characterise the prevalence and temporal patterns of single AE following LCIG initiation; secondly, to identify clinical predictors of treatment discontinuation.

METHODS: Baseline demographic, clinical, electrophysiological, and treatment data were collected in a single-centre retrospective study of aPD patients treated with LCIG. AEs were assessed at baseline and systematically every six months thereafter. Treatment persistence and predictors of discontinuation were analysed using Kaplan-Meier estimates, Cox proportional hazards models.

RESULTS: 117 patients were included (mean disease duration: 14 ± 5 years; mean follow-up: 33.8 ± 31.9 months). LCIG discontinuation occurred in 26% of patients and 26% died). Early dropout (<1 month) occurred in 10.6%, mainly due to acute medical or device-related complications. Cumulative incidence analyses revealed distinct temporal patterns according to AE type. Dyskinesia and PNP showed a rapid early increase, whereas hallucinations, device-related complications, and weight loss accumulated progressively throughout follow-up. Confusion, abdominal pain, and OH increased mainly during the first years of treatment before reaching a plateau. 22% of the patients discontinued the treatment due to AEs. Lower cognitive performance and dyskinesia> 1 h/day were associated with a higher cumulative incidence of treatment discontinuation.

CONCLUSIONS: In real-world practice, single AE may follow different cumulative incidence trajectories, with device-related complications, hallucination and weight loss showing a progressive incidence over long follow-up. AEs related to treatment discontinuation occur in about one-fifth of the patients.
