Upper Extremity Vascular Access: Challenges, Controversies, and Clarifications.
Аннотация
Upper extremity arterial access is increasingly used in cardiovascular procedures because it offers improved patient safety, comfort, and faster recovery than the traditional femoral (groin) approach. This approach has clear benefits but also some challenges: the radial artery can become occluded or spasm, and its smaller size can limit equipment options. Fortunately, these risks can be minimized by proper technique-giving adequate heparin, maintaining blood flow during compression (patent hemostasis), using vasodilators to prevent spasm, employing ultrasound guidance, and working with thin, low-friction sheaths. Concerns that radial access might increase radiation exposure, stroke risk, or reduce success rates have been dispelled by evidence. Dialysis patients and those with coronary bypass grafts need special planning, since their arm vessels may be required for fistulas or grafts in the future. In select cases, radial access can still be used while safeguarding these patients' future vascular options.
