Аннотация
This poetic reflection highlights how routine bedside tests become opportunities for multifaceted learning for trainees. The "finger-to-nose" test used in neurologic examinations not only provides insight into neurologic signs but also teaches about clear communication, patience, and connecting with the patient's way of thinking. Drawing on real-life, day-to-day clinical encounters, the piece explores the patient's often misinterpreted instructions and the unexpected responses they provoke as the trainee learns to administer the neurologic examination.I say it gently, as a part of my examination,"Touch your nose, then my finger."Time seems to hold still,While I brace to expect the unexpected. The patient's path meanwhile, was already underway,With whatever they thought they heard me say.A pauseA furrowed brow. Then the first unexpected question rises like mist:Whose nose? His? Mine? The patient stares, and then decides to act rather than ask. Some aim true, and find their nose, Others explore the neighbourhood: lip, cheek or chin,But the truly daring take aim at mine,A misfire, I have learnt to intercept, with time. Then begins the journeyFinger to finger, there and back again. As I hold my finger still, like a lighthouse in fog, And I watch. Not just for the tremor, Not the possibly perilous past-pointing directed at my nose, Or the tell-tale dance of a drunken neuron. But for that moment when chaos and comprehension meet, and the fingers touch. The finger lands. And then the nose is touched. And somewhere deep in the cerebellum,A neuron sings.
