Аннотация

BACKGROUND

Free toe pulp transfer is an established microsurgical option for fingertip reconstruction. However, outcome reporting remains heterogeneous, and no focused synthesis integrating flap survival with functional, sensory, and complication outcomes is currently available.

METHODS

This review was conducted according to PRISMA guidelines. PubMed, Embase, Web of Science, and Scopus were searched on January 21, 2026. Studies reporting outcomes after microsurgical free great toe or second toe pulp flap transfer for fingertip, thumb pulp, or distal digital soft-tissue defects were included. Quantitative synthesis was limited to flap survival, for which a random-effects meta-analysis of proportions using Freeman-Tukey transformation was performed. Other outcomes were synthesized descriptively because of heterogeneity in definitions and reporting.

RESULTS

Seventeen studies comprising 1300 flaps were included. Total flap loss occurred in 9 cases (0.69%) and partial necrosis in 5 (0.4%). The pooled survival rate was 99.8% (95% CI 99-100%), with low heterogeneity (I² = 22.5%). Sensory recovery was variable but generally favorable, with reported mean static two-point discrimination ranging from 3.7 to 13.1 mm. Cold intolerance was reported in 42 of 160 assessed patients (26.3%), although reporting was heterogeneous. Esthetic and patient-reported outcomes were generally satisfactory when assessed.

CONCLUSIONS

Free toe pulp flaps appear to be a reliable reconstructive option for fingertip defects, with consistently high survival rates and generally favorable reported outcomes. However, the current evidence is limited by retrospective study designs, heterogeneous outcome reporting, and the disproportionate influence of a single high-volume series. Further prospective comparative studies with standardized outcome measures are required.
