Аннотация

ISSUE ADDRESSED: No Vax, No Visit (NVNV) is a parent-led socially negotiated health protection practice aimed at reducing newborns' risk of exposure to pertussis by encouraging visitors to be up to date with tetanus-diphtheria-pertussis (Tdap/dTpa) vaccination prior to contact. While consistent with Australian cocooning guidance, NVNV shifts responsibility for communication and enforcement onto parents. Its psychosocial impacts and health promotion implications remain poorly understood.

METHODS

A cross-sectional mixed-methods online survey was conducted with Australian parents of children under 5 years (n = 166). Measures included NVNV awareness and adoption, visitor resistance, psychological distress (K10), and social connectedness. Quantitative data were analysed using descriptive statistics, χ2 tests, t-tests, ANOVA, and regression models. Open-text responses (n = 456; 152 participants) were analysed using directed content analysis informed by the WHO Behavioural and Social Drivers of Vaccination framework and the socioecological model.

RESULTS

NVNV adoption was high (80.1%), and healthcare provider endorsement and lower vaccine hesitancy were strong predictors of uptake. Over one quarter of parents (27.7%) reported resistance from relatives or close contacts. Parents who experienced resistance reported significantly higher psychological distress than those who did not. Qualitative findings revealed emotional labour, interpersonal conflict, and reduced practical and emotional support when vaccination was declined.

CONCLUSIONS

NVNV is widely adopted but can generate psychosocial strain when parents encounter resistance, with distress driven by interpersonal conflict rather than NVNV adoption itself. SO WHAT?: Health promotion should move beyond individualised responsibility by providing consistent public health messaging on contact vaccination, communication tools for parents, and equitable access to adult immunisation services, supporting families to protect newborns without compromising wellbeing or social support.
