Hemorrhagic posterior vitreous detachment: A comprehensive review of the risks of retinal breaks, detachment, and management options.
Аннотация
Patients with hemorrhagic posterior vitreous detachment (hPVD), defined as concomitant PVD and vitreous hemorrhage, are at heightened risk of retinal breaks (RBs) and detachment (RD). Despite this, patients are routinely given the option between conservative management and early intervention with vitrectomy, as no consensus has been reached regarding the superiority of either strategy. Herein, we provide a scoping review of the current evidence regarding the risk of RBs and RD and management of patients with hPVD. Findings suggest that 30-60% of patients with hPVD may suffer from retinal breaks and are therefore at risk of retinal detachment. Several studies, however, report no appreciable differences in visual outcomes between patients with hPVD that are treated with early surgery and those managed conservatively. Ultimately, various factors may impact management strategy in this clinical context, and we provide a nuanced summary of pertinent factors.
