INTRODUCTION: The therapeutic landscape of hemophilia A has expanded beyond factor replacement with the emergence of non-biologic pharmacological strategies that modulate endogenous hemostatic pathways. These innovations challenge traditional treatment paradigms and create opportunities for more individualized approaches to hemostatic management.
AREAS COVERED: This narrative review examines the evolution of non-biologic pharmacotherapy, focusing on RNA therapeutics, emerging oral small molecules, and future mechanism-based interventions that restore hemostasis by rebalancing coagulation rather than by factor VIII replacement. Literature was identified through searches of PubMed/MEDLINE, Embase, ClinicalTrials.gov, and major hemostasis conference proceedings, and was supplemented by reference screening and screening of regulatory guidance websites. Evidence was critically appraised for scientific quality, mechanistic insight, and clinical relevance. We discuss advances in systems pharmacology, functional biomarkers, combination pharmacology, and implementation science, and introduce Precision Coagulation as a framework for the individualized restoration of hemostatic equilibrium, integrating clinical, laboratory, pharmacodynamic, and biological information.
EXPERT OPINION: Non-biologic therapies represent more than new treatment options; they redefine hemophilia pharmacotherapy by establishing hemostatic modulation as a therapeutic principle. Future management will increasingly depend on biomarker-guided, mechanism-based, and patient-centered strategies that integrate biologics, non-biologic therapies, and emerging technologies.
