Dorsal preservation rhinoplasty is becoming more widely used due to the ability to preserve the dorsal bony-upper lateral cartilage junction, the dorsal septum to the ethmoid, and nasal ligaments. While this technique offers some advantages compared to other rhinoplasty techniques, it is important to assess patients' candidacy for dorsal preservation techniques preoperatively to avoid surgical complications. This article discusses key considerations when performing dorsal preservation rhinoplasty including identifying blocking points. The authors also discuss their revision techniques for addressing complications in dorsal preservation rhinoplasty.
